Preparing for your Appointment
In order for us to provide the best care for you, it is very important that you bring the following items to your visit:
* A list of your current medications
* Any and all x-rays, MRIs or other imaging films (including written report)
* Insurance information and card
* Information about what treatments and medications have been tried in the past
* Medical records from prior visits that you think may be helpful
* Referral from your primary care physician, if applicable
* Co-payment or minimum payment (given at appointment time) for uninsured patients
* List of questions you want to ask your physician
Scheduling an Appointment:
Premier Orthopedic & Neurosurgical Group offers its clients a few ways to Schedule an Appointment with us; You may call our Central Scheduling at 1.888.575.0007 during regular business hours. You may also call the office which you would like to see the doctor in directly and the last option is using the Online Scheduling form and one of our Staff members will call you be within 24-48 hours. When scheduling an appointment, the receptionist will gather information to ensure enough time is allocated for your visit. We have the utmost respect for everyone's time, and we do everything we can to make the best use of both your time and your physician's time.
Please be prepared with the following information:
* Your name
* Your date of birth
* Brief description of the reason for the appointment
* Your address
* Home and alternate (work) phone numbers
* Your primary physician or health care provider
* Your insurance information
Canceling an Appointment:
Cancellations should be made at least 24 hours in advance of your scheduled appointment time. If you discover that you will not be able to keep your appointment, please contact our office as soon as possible to reschedule your appointment. Renewing Prescriptions:
Please call Central Scheduling, 1.888.575.0007 for more information. Please call your pharmacy to renew your prescription. They will contact us for any necessary information they may need. Please allow at least 48-72 hours to accommodate your request, as each request has to be approved by the physician who is not in the office every day. If your prescription is running low, please do not wait for the weekend or until you have completely ran out of your medication.
